Abstract
Latar Belakang: Kanker payudara merupakan jenis kanker tersering dan menyebabkan kematian pada wanita. Salah satu pernyebab kematian kanker payudara adalah metastasis. Diperkirakan 5-10% kanker payudara sudah mengalami metastasis saat didiagnosis awal dengan stadium lanjut. Metastasis dipengaruhi oleh berbagai faktor, oleh karena itu belum diketahuinya faktor risiko apa saja yang berpengaruh terhadap kejadian metastasis pada pasien kanker payudara stadium lanjut di RS Ibnu Sina Kota Makassar.
Tujuan: Menganalisis berbagai faktor risiko yang berpengaruh terhadap kejadian metastasis pada pasien kanker payudara stadium lanjut di RS Ibnu Sina Kota Makassar.
Metode: Penelitian observasional analitik menggunakan desain cross-sectional dengan jumlah sampel 120 responden pada pasien kanker payudara stadium lanjut III dan IV. Variabel bebas adalah usia melahirkan anak pertama, status menopause, stadium TNM, grade histopatologi, diabetes mellitus tipe 2, anemia dan hipertensi. Variabel terikat adalah kejadian metastasis. Faktor risiko dengan analisis bivariat menggunakan uji chi-square dan analisis multivariat menggunakan metode regresi logistik.
Hasil: Faktor yang berpengaruh terhadap kejadian Metastasis adalah stadium TNM (T) (p=0,007) PR 5,049 (1,572-16,217), stadium TNM (N) (p= 0,018) PR 3,726 (1,254-11,073), grade histopatologi (p=0,000) PR 8,713 (2,877-26,387), diabetes mellitus tipe 2 (p= 0,000) PR 8,713 (2,877-26,387), dan anemia (p=0,016) PR 4,584 (1,321-15,899). Faktor risiko yang tidak berpengaruh adalah usia, usia melahirkan anak pertama, status menopause, dan hipertensi.
Kesimpulan: Stadium TNM, grade histopatologi, diabetes mellitus tipe 2, dan anemia merupakan faktor yang berpengaruh terhadap terhadap kejadian metastasis dengan probabilitas 84%.

Background : Breast cancer is the most common type of cancer and causes death in women. One of the causes of breast cancer death is metastasis. It is estimated that 5-10% of breast cancers have metastasized when diagnosed early with an advanced stage. Metastasis has influenced by several factors, therefore didn’t know what risk factors affect the incidence of metastasis in patients with advanced breast cancer at Ibnu Sina Hospital, Makassar.
Objective: To analyze the various tisk factors that influence the incidence of metastasis in patients with advanced breast cancer at Ibnu Sina Hospital, Makassar.
Methods: This observational analytic study used a cross-sectional design with a sample size of 120 respondents in patients with advanced stage III and IV breast cancer. The independent variables were age at first birth, menopausal status, TNM stage, histopathological grade, diabetes mellitus type 2, anemia and hypertension. The dependent variable is the occurrence of metastasis. Risk factors with bivariate analysis using the chi-square test and multivariate analysis using logistic regression methods.
Results: Factors that influence the incidence of metastasis are the TNM stage (T) (p = 0.007) PR 5.049 (1.572-16.217), TNM stage (N). (p = 0.018) PR 3,726 (1,254-11,073), histopathological grade (p = 0,000) PR 8,713 (2,877-26,387), type 2 diabetes mellitus (p = 0,000) PR 8,713 (2,877-26,387), and anemia (p = 0.016) PR 4.584 (1.321-15,899). Risk factors that did not affect were age, age at first birth, menopausal status, and hypertension.
Conclusion: TNM stage, histopathological grade, type 2 diabetes mellitus, and anemia are factors that influence the incidence of metastases with a probability of 84%.
