Abstract
Introduction: Preterm labor accounts for 75% of perinatal deaths. Infection and activation of the inflammatory response driven by infection are considered major risk factors for spontaneous preterm labor. DHA as a precursor of docosanoids (resolvins, maresins) has anti-inflammatory properties and protects individuals from inflammation.
Aim: Analyzing the relationship between serum DHA levels and the incidence of premature birth.
Method: The croos sectional study involved 40 pregnant women with preterm labor between >20 - <37 weeks of gestation and 40 normal pregnant women with gestation between >20 - <37 weeks of gestation. Evaluation was conducted on maternal venous blood DHA levels obtained within <24 hours after delivery. Analysis was conducted using the Independent T-test, Mann Whitney U, Chi-square and Fisher exact tests with a significance of p<0.05.
Result: DHA levels of preterm labor subjects had an average of 82.32 ± 60.05 μg/mL while normal subjects had an average of 117.61 ± 85.55 μg/mL with a significant difference between study groups (p = 0.004). ROC curve analysis (AUC 0.688, p = 0.002) of DHA levels obtained a cut off of <89.07 μg/mL with a sensitivity of 70% and a specificity of 62.5%. Pregnant women with DHA levels <89.07 μg/mL had a 2x higher risk (RR 2.00; 95% CI: 1.20-3.35; p = 0.04) of experiencing preterm labor compared to pregnant women with DHA levels >89.07 μg/mL.
Conclusion: Evaluation of DHA levels during pregnancy can be used as a predictor of the incidence of premature birth.
