Abstract

Patients with chronic kidney disease undergoing hemodialysis often require long-term care provided by family members acting as informal caregivers. Continuous caregiving responsibilities may lead to caregiver burden, which can adversely affect caregivers' quality of life. One protective factor that may support caregivers is family resilience. This study aimed to examine the mediating role of family resilience in the relationship between caregiver burden and the quality of life of caregivers of hemodialysis patients. A quantitative approach with total sampling was employed, involving 75 informal caregivers of hemodialysis patients at Teungku Peukan Regional General Hospital, Southwest Aceh. Three instruments were used in this study: the Family Resilience Scale adapted from Herfinanda (2022), the Quality of Life Scale adapted from Purba et al. (2018), and the Caregiver Burden Scale adapted from Larantukan and Yudiarso (2025), with several items modified based on previous psychometric evaluations. Data were analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM) with SmartPLS 4 software. The results indicated that caregiver burden was significantly associated with caregivers' quality of life, with a path coefficient of -0.555 and a p-value of 0.000 (<0.05). However, the relationship between caregiver burden and family resilience was not supported, with a path coefficient of 0.355 and a p-value of 0.157 (>0.05). Similarly, the relationship between family resilience and caregivers' quality of life was not significant, with a path coefficient of -0.200 and a p-value of 0.416 (>0.05). Family resilience also did not significantly mediate the relationship between caregiver burden and caregivers' quality of life, as indicated by a path coefficient of 0.071, a p-value of 0.537 (>0.05), and an upsilon V effect size of 0.005, suggesting a negligible mediating effect. These findings indicate that although family resilience is an important factor in facilitating family adaptation to chronic illness, its role as a mediating variable in this relationship could not be empirically supported in the present sample. In conclusion, caregiver burden is an important factor that warrants attention in efforts to improve the quality of life of caregivers of hemodialysis patients.
