Abstract

Background:The increasing elderly population is a global challenge that impacts health care systems, particulary in maintaining the independence of older adults in performing Activity of Daily Living (ADL). Japan and Indonesia both face population aging, however, they differ in their long term care systems and support for elderly independence. Understanding elderly care practices in Japan is essential as a learning reference for developing elderly care services in Indonesia. Methods: This Study employed a qualitative approach using primary and secondary data sources. Primary data were obtained through observations and interviews with nurses, caregivers, and care managers at Hospital 1, Hospital 2, and three elderly care facilities in Kagoshima, Japan. Secondary data were devided from the Kaigo Hoken (Long Term Care Insurance) book of Kagoshima City, Reiwa 7 (2025) edition. Results: The results indicate that the fulfillment of ADL among older adults in Japan is supportes by a well structured Long Term Care Insurance (LTCI) system, which includes regular ADL assessment and reassessment, availability of assistive devices, environmental modifications, and active involvement of nurses and caregivers. Older adults are encouraged to continue performing daily activities according to their functional abilities to mantain independence. Conclusion: Japan’s elderly care system provides valuable lessons in supporting independence in ADL among older adults. The implementation of LTCI, including periodic independence assessments, provision of assistive devices, and supportive facilities, can serve as a reference for improving elderly care service in Indonesia
