Abstract

Latar Belakang: Negara Indonesia menempati peringkat tertinggi ketiga di dunia dengan beban penyakit menular khususnya Tuberkulosis. Kota Palu merupakan kota/kabupaten yang memiliki kasus Tuberkulosis tertinggi di Sulawesi Tengah. Peningkatan kasus tuberkulosis dalam lima tahun terakhir di Kota Palu, salah satunya disebabkan oleh banyaknya pasien tuberkulosis paru yang mengalami putus berobat sehingga memungkinkan meluasnya penularan kuman Mycobacterium tuberculosis di lingkungan. Ketidakpatuhan pengobatan terjadi akibat dipengaruhi oleh beberapa faktor salah satunya self efficacy penderita dan faktor hambatan serta dukungan lainnya.
Tujuan: Penelitian ini bertujuan untuk menganalisis hubungan self efficacy penderita tuberkulosis paru dan faktor hambatan serta dukungan lainnya dengan kejadian putus berobat di Kota Palu Provinsi Sulawesi Tengah.
Metode: Jenis penelitian ini menggunakan metode kuantitatif dengan desain case control yang dilakukan di Kota Palu mulai Juni hingga Agustus 2023. Penelitian ini terdapat sampel sebanyak 108 responden dan data diolah menggunakan SPSS dengan uji chi square.
Hasil: Hasil dari penelitian ini menunjukkan bahwa pengalaman diri sendiri yang gagal (OR=34,025;95%CI=5,979-193,640;p=<0,001), pengalaman keberhasilan orang lain yang gagal (OR=20,271; 95%CI=3,663-112,189;p=0,001), tidak mendapatkan persuasi verbal dari lingkungan (OR=10,504;95%CI=1,589-69,453;p=0,015) serta kondisi fisik dan emosional yang tidak baik (OR=12,713;95%CI=2,512-64,345,p=0,002) merupakan faktor risiko dengan kejadian putus berobat pasien tuberkulosis paru di Kota Palu. Hasil regresi logistik menunjukkan bila pengalaman diri sendiri yang gagal, pengalaman keberhasilan orang lain yang gagal, tidak mendapatkan persuasi verbal dari lingkungan dan kondisi fisik dan emosional yang tidak baik kemungkinan mengalami putus berobat 98,8%.
Simpulan: Pengalaman diri sendiri yang gagal, pengalaman keberhasilan orang lain yang gagal, tidak mendapatkan persuasi verbal dari lingkungan dan kondisi fisik dan emosional yang tidak baik memiliki hubungan dengan kejadian putus berobat pasien tuberkulosis paru di Kota Palu Provinsi Sulawesi Tengah.

Background: Indonesia has the third highest burden of infectious diseases in the world, especially Tuberculosis. Palu City is the city/district with the highest number of tuberculosis cases in Central Sulawesi. The increase in tuberculosis cases in the last five years in Palu City was caused by the number of pulmonary tuberculosis patients who discontinue of treatment, allowing the spread of Mycobacterium tuberculosis germs in the environment. Treatment non-compliance occurs as a result of being influenced by several factors, one of which is a patiet's self-efficacy and other barriers and support factors.
Objective: This study aims to analyze the relationship between self-efficacy of pulmonary tuberculosis patients and other barriers and support factors with the incidence of discontinuation of treatment in Palu City, Central Sulawesi Province.
Methods: This type of research uses quantitative methods with a case-control design conducted in Palu City from June to August 2023. This study had a sample of 108 respondents, and the data were processed using SPSS with the chi-square test.
Results: The results of this study showed that the variables of the failure of self-experience (OR=34,025;95%CI=5,979-193,640;p=<0,001), the failure of the success from others (OR=20,271; 95%CI=3,663-112,189;p=0,001), not getting verbal persuasion from environment (OR=10,504;95%CI=1,589-69,453;p=0,015), and poor physical and emotional conditions (OR=12,713;95%CI=2,512-64,345,p=0,002) are risk factors with the incidence of treatment discontinuantion in pulmonary tuberculosis patients in Palu City. The results of logistic regression showed that the failure of self-experience, the failure of the success from others, not getting verbal persuasion from environment and poor physical and emotional conditions are the possibility of discontinuation of treatment is 98,9%.
Conclusion: The failure of self-experience, the failure of the success from others, not getting verbal persuasion from environment and poor physical and emotional conditions are related to the incidence of discontinuation of treatment for pulmonary tuberculosis patients in Palu City, Central Sulawesi Province.
