Abstract

Background: Lymphatic metastases are common among patients with endometrial carcinoma (EC), and their presence reduces the patient's chance of survival. Several consensuses advocate lymphadenectomy before the metastasis is detected, whereas others doubt its therapeutic benefits due to the morbid complications. Thus, any clue that affects the decision to perform lymphadenectomy becomes crucial. This study aims to compare the prevalence of pelvic lymph node (PLN) metastasis in low-, medium-, and high-risk early-stage EC patients.
Material and methods: This is a retrospective cross-sectional study of 177 women diagnosed with endometrial carcinoma (EC) who underwent lymphadenectomy and surgical staging between 2017 and 2021 in Dr. Kariadi General Hospital, Semarang. We focused on the presence of PLN and risk categories of early-stage EC (low, intermediate, and high-risk), which were determined from the medical records. The correlation between the presence of PLN metastasis and the risk categories of EC was assessed using the Chi-square test.
Results: There is a statistically significant difference in the prevalence of PLN metastases of early-stage EC between low, intermediate, and high-risk groups (p=0,013). The highest prevalence of PLN metastases (9.2%) was recorded in intermediate-risk early-stage EC, whereas no PLN metastases were detected in low and high-risk early-stage EC.
Conclusion: The prevalence of PLN metastases differs significantly among risk categories of early-stage EC before surgical staging. The highest prevalence of PLN metastases was found in intermediate-risk early-stage EC.
