Abstract
Latar belakang: Infeksi saluran kemih (ISK) merupakan penyakit infeksi urutan kedua terbanyak di dunia. Sebesar 75-90% ISK disebabkan oleh bakteri Escherichia coli. Pemakaian antibiotika beta-lactam yang tidak sesuai dapat menyebabkan terjadi resistensi terhadap antibiotika tersebut. Salah satu resistensi adalah resistensi terhadap extended spectrum beta lactamase (ESBL).
Tujuan: Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan status bakteri penghasil Extended Spektrum Beta Laktamase pada pasien ISK di RSUD dr. R. Soetijono Blora.
Metode: Sebuah studi Cross sectional ini telah dilaksanakan RSUD dr. R. Soetijono Blora pada bulan Januari-Maret 2023. Berdasarkan uji antibiotik diperoleh sebanyak 39 orang (67,2%) ESBL dan 19 orang (32,8%) Non-ESBL. Variabel bebas meliputi usia, jenis kelamin, riwayat pemasangan kateter, riwayat batu atau operasi saluran kemih, riwayat penggunaan antibiotik dalam 6 bulan terakhir dan riwayat diabetus mellitus. Variabel terikat yaitu status bakteri penghasil ESBL pasien ISK. Pengumpulan data menggunakan hasil uji antibiotik dan kuesioner, sedangkan analisis data menggunakan uji chi square dan uji regresi logistic.
Hasil : Diketahui faktor-faktor yang hubungan yang bermakna dengan status bakteri penghasil ESBL adalah riwayat pemasangan kateter (p=0.008) dan riwayat penggunaan antibiotik dalam 6 bulan terakhir (p=0.015). Dan faktor yang tidak ada hubungan dengan status bakteri penghasil ESBL adalah jenis kelamin (p=0.697), usia (p=1.000), riwayat rekurensi ISK (p=0.136), riwayat batu atau operasi saluran kemih (p=0.718) dan riwayat diabetes melitus (0.276).
Kesimpulan: Prevalensi bakteri penghasil ESBL pada pasien ISK di RSUD dr. R. Soerijono Blora tinggi dan salah satu faktor yang dapat meningkatkan risiko seseorang terkena infeksi bakteri penghasil ESBL yaitu riwayat pemasangan kateter.

Background: Bladder tract infection (UTI) is the second most common infectious disease in the world. 75-90% of UTIs are caused by Escherichia coli bacteria. Inappropriate use of beta-lactam antibiotics can lead to resistance to these antibiotics. One of the resistances is resistance to extended spectrum beta lactamase (ESBL).
Purpose: This study aims to determine the factors associated with the status of Extended Spectrum Beta Lactamase producing bacteria in UTI patients. at RSUD dr. R. Soetijono Blora.
Methods: A cross-sectional study has been carried out at RSUD dr. R. Soetijono Blora in January-March 2023. Based on the antibiotic test, 39 people (67.2%) were ESBLs and 19 people (32.8%) were non-ESBLs. Independent variables included age, sex, history of catheter insertion, history of stones or urinary tract surgery, history of using antibiotics in the last 6 months and history of diabetes mellitus. The dependent variable is the status of ESBL-producing bacteria in UTI patients. Data collection used the results of antibiotic tests and questionnaires, while data analysis used the chi square test and and logistic regression test.
Results: It is known that factors that have a significant relationship with the status of ESBL-producing bacteria are history of catheter insertion (p=0.008) and history of use of antibiotics in the last 6 months (p=0.015). And factors that had no relationship with the status of ESBL-producing bacteria were gender (p=0.697), age (p=1.000), history of UTI recurrence (p=0.136), history of stones or urinary tract surgery (p=0.718) and history of diabetes mellitus (0.276).
Conclusion: The revalence of ESBL-producing bacteria in UTI patients at RSUD dr. R. Soerijono Blora is tall and one of the factors that can increase a person's risk of developing an ESBL-producing bacterial infection is a history of catheter insertion.
